T0 – Initial generic case investigation form

  • When the disease/exposure causing the outbreak is unclear, in the first hours or days following an alert, use the T0 (T”0” stands for time 0) to collect patient data.  
  • When there is no specific case investigation form to investigate the event available, because of other than infectious hazards or new/emerging disease.

SYNDROME Case investigation form

  • When the disease causing the outbreak is unknown and the clinical syndrome has been identified.

EXPOSURE case investigation form

  • When the disease causing the outbreak is unknown and the source or mode of transmission is suspected.

T1 - Unknown disease case investigation form

  • When the disease, syndrome and/or the route of transmission are undetermined.

     

Disease case investigation form(s)

  • When the disease is confirmed or suspected.
  • When WHO does not have a case investigation form, we propose case investigation forms prepared by partner institutions.
